Purchasing Policies

 

PRIVACY POLICY:

We respect and are committed to protecting your privacy. We may collect personally identifiable information when you visit our site. We also automatically receive and record information on our server logs from your browser including your IP address, cookie information, and the page(s) you visited. We will not sell your personally identifiable information to anyone. Any online payments and purchases are also processed with a secure and encrypted line to safeguard your information. If your purchase is regulated by HIPAA we abide by this policy.

SECURITY POLICY:

Your payment and personal information we collect for paying are always safe. Our Secure Sockets Layer (SSL) software is the industry standard and among the best software available today for secure commerce transactions. It encrypts all of your personal information, including credit card number, name, and address, so that it cannot be read over the internet.

REFUND POLICY:

Accounts will be reviewed by a collector to determine if a refund is due to either patient or client. The Director of Patient Financial Services will also review for accuracy. Refunds greater than $1000.00 will be reviewed by the CFO. Once the refund is approved, it will be given to Madison Memorial Hospital’s Accounts Payable to process the check. You may receive it by mail thereafter.

SHIPPING POLICY/DELIVERY POLICY:

Please be assured that your items will ship out within a week of purchase. We determine the most efficient shipping carrier for your order. The carriers that may be used are the U.S. Postal Service (USPS), United Parcel Service (UPS), or FedEx.
