Upon completion of the training and the form below, bring your current immunization records (Measles, Mumps, Rubella, Tuberculosis Screening, Tdap, and influenza documentation) to meet in person with the volunteer services secretary. Current immunizations improve Madison’s infection control efforts.

You will be required to complete additional paperwork in person. You will need to know your Social Security Number and other pertinent information required for your background screening.

Your externship status is dependent upon submitting immunization information and an approved background check.
