Patient Portal gives you secure online access to some of your health information found at Madison Memorial Hospital. This will allow you to access your information whenever you have internet access. If you’d like to access information from recent visits, after July, click on the first link. If you are a accessing visits from before August, click on the second link:
1. Visit madisonmemorial.org
2. Click on patient portal icon
3. Complete Enrollment Questionnaire
4. Access your email and retrieve your one time user name, password, and link to the portal
5. Click on link to access portal
Enter one-time user name, password, and security question, then click “Log On”
6. Enter your new user name and password
7. Enjoy the Portal
Is there a fee to use patient portal?
No. The Patient Portal is a free service offered to our patients.
What do I need to do to use Patient Portal?
You will need access to a computer with internet access and an updated browser (e.g. Internet Explorer, Chrome).
Where does the portal information come from?
All of the information in the Portal comes from Electronic Health Records at Madison Memorial Hospital. This ensures that you have access to the most accurate, up-to-date information possible.
Is my information safe?
Yes. Portal passwords are encrypted and url’s are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.
