To see your lab results for Covid testing, or anything else, simplify your life through the patient portal. Madison Patient Portal gives patients secure and convenient, 24-hour access to personal information and medical records for patients who have had a visit to Madison or one of our clinics since August of 2017.

Our Patient Portal gives you secure online access to some of your health information found at Madison. This will allow you to access your health information by clicking the Patient Portal button below whenever you have internet access. If you can’t find what you are looking for contact the Health Information Management (HIM) Department.

Madison Memorial is dedicated to providing resources to help you better manage your health. We are excited to announce that our Patient Portal works with the Apple Health app, which is available to patients with an iPhone running iOS 11.3 or later.

APPLE HEALTH APP SETUP

The setup is simple. Accessing your health records from Madison Memorial and its clinics through your iPhone is as easy as

    • going to the Health Data section and selecting Health Records on the Health app on your iPhone,
    • add an account and search for a Madison Memorial Health Partners,
    • and then log in with the user name and password that you normally use for Madison Memorial Patient Portal

After you log in once, your health records will start to appear in the Health app, and you will be notified when new records are ready to view.

Under Idaho State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without approval from the health care provider or consent of the minor patient. Child’s information (12 years or older) must be requested through the Health Information Management (HIM) Department.

If you’re aware of an application to review health information that is not available at Madison you can reach out to the developer and direct them to https://code.cerner.com/.
