If your admission has been scheduled in advance, you may pre-admit by phone or in person at the hospital admitting office, located near the main entrance.
If your visit is an emergency or if it is after 5:30 pm, admissions are processed at the Emergency Department desk next to the main entrance.
During your admission process, you will be asked to provide your insurance card and to sign several consent forms. You will be given an identification bracelet, and we ask that you wear this bracelet at all times. Your room location will be assigned according to the medical care you are to receive.
In addition to bringing your insurance card, please have your claim forms, social security number, driver’s license, and any other pertinent information. We ask that you bring a list of your medications and include all of the information on the prescription label. Please, do not bring the medication unless your doctor specifically asks you to.
What to Bring
You will need only a few personal items for your hospital stay, such as a nightgown or pajamas, a bathrobe, nonskid slippers, and personal care items like a toothbrush, toothpaste, a comb, and shampoo.
It is against safety regulations to bring any electric appliances into the hospital such as radios, hair dryers, electrical shavers, and coffee pots. Please do not bring jewelry or other valuables. The hospital does not accept responsibility for the loss of personal items.
When not in use, dentures, hearing aids, and eyeglasses should be placed in protective containers since the hospital cannot be responsible for the breakage or loss of such articles.
